"On October 14, 2021, 28-year-old Karina Hernandez (my fiance) went to Dr. Eugene G. Pereira for a intrathecal injection. Prior to the procedure, Karina Hernandez asked Dr. Pereira whether I (Jeremiah Lee Grayson) could accompany her. We were told that I could not go with her. A few minutes later, Dr. Pereira told me to go to the room where the procedure was being done so that I could calm Karina down. I noticed Karina was in pain when I entered the room, so I grabbed her right hand and asked her what was wrong. Karina stated that she could feel the needle on her neck. I checked her neck and let her know that there wasn’t a needle there. She then started to let go of my hand as her eyes rolled back. Dr. Pereira stated that he would seek assistance from someone to turn Karina over. I turned Karina over by myself because she was nonresponsive or breathing, and I didn’t want to risk her life. Dr. Pereira returned alone a few minutes later. He walked me back to the room I was waiting in and closed the door. I opened the door to see what was happening. Another doctor showed up and closed the door again. I opened the door again and left it open to see if she was getting CPR. I did not see anyone perform CPR on Karina, Dr. Pereira failed to appropriately respond to Karina’s airway emergency. EMS found Karina dead without resuscitation efforts when they arrived at the facility. She went 10 or more minutes without oxygen, leading to her hypoxic injury. Additionally, Dr. Pereira deleted important fluoroscopy images used during the procedure. Karina was transferred to Emory Decatur where she spent the following 8 months in a vegetative state until she ultimately passed away on June 16, 2022."

"WOW! I have never been so insulted by a doctor and her nurse in all my life! Nurse very disrespectful and mouthy. I got called dumb and stupid because I did not agree with the doctors plan for me. She would not allow me to voice my opinion or have any say, and paid no attention to my concerns about her plan. Doctor Johnson would not even consider lifestyle /plant based foods and exercise. She based her entire decision on a 4 year old history, prescribed 3 different kinds of meds right away, one which was potassium. I am on a plant based diet and everything I eat has potassium in it. Too much potassium in the blood is called hyperkalemia and can be very dangerous. She refused to go over the blood test that she ordered, which was two weeks after she ordered the meds. And when I asked for the blood tests, her mouthy nurse asked me if I could read the tests. 12 times in two visits I told both doctor Johnson and her nurse that I was no longer taking supplements ( but she was only looking at the 4 year history, and paid no attention to what I kept telling her) Both the doctor and the nurse made very unkind remarks about my hearing loss. I am deaf, not dumb. And she made remarks about my using a transcriber on my phone. My feelings about this doctor: A close minded doctor, such as Dr. Johnson, is a dangerous doctor. "
