Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Spinal Cord Injury

Spinal cord injury medicine is a branch of medicine that treats damage to the spinal cord from an injury or a nontraumatic myelopathy, which is any disorder that affects the spinal area. The spinal cord is a section of nerve tissue protected by the vertebrae (spinal bones). Trauma to the spinal cord, typically caused by compression or bent vertebrae, prevents the body and brain from communicating. This can result in pain, loss of sensation, and impaired physical function and movement. Acute spinal cord injuries (SCIs) are a leading cause of permanent disability. Spinal cord injuries may develop from accidents or myelopathies, disorders that provoke spinal compression. Traumatic incidents such as falls, sports injuries, vehicle accidents, and bullet or stab wounds can cause an acute spinal cord injury.

Spinal cord injury medicine seeks to stabilize the spine and to alleviate the symptoms of spine damage. Practitioners of spinal cord injury medicine are called spinal cord injury specialists. Spinal cord injury medicine is an interdisciplinary field, meaning physicians of various specialties may be trained to treat spinal cord injuries.

Damage to the spinal cord may initially be diagnosed through X-ray, MRI, or CT scans. Patients can also undergo spinal exams to evaluate their sensory ability and strength. Injuries to the spinal cord can be complete (no feeling or sensation) or incomplete (some feeling or sensation remains). Conditions spinal cord injury medicine specialists may treat include:

  • Paralysis, such as paraplegia (loss of function in the legs), or quadriplegia (loss of function from the neck down)
  • Spine fracture or trauma, which can lead to impairment and/or significant pain
  • Spinal infections
  • Myelopathies, such as herniated disks, spinal tumors, and rheumatoid arthritis
  • Inflammatory diseases such as ankylosing spondylosis, and degenerative conditions like Lou Gehrig's disease (ALS)

Treatment for spinal cord injuries often involves surgery, either directly after an injury occurs or at a later date. Surgery for spine injuries is intended to ease spinal compression and stabilize the spine. Surgical procedures can involve shifting vertebrae, removing bone, or altering spinal placement with implantable devices. Patients with significant spinal trauma may require urgent surgical intervention.

Surgical treatments can be complemented by rehabilitative physical therapy to improve mobility. Spinal cord injury specialists also treat complications arising from a spinal injury, such as respiratory or bladder conditions. Some patients with spinal cord injuries may require lifelong treatment. Spinal cord injury specialists aim to help these patients successfully adjust and lead an improved quality of life. An emerging treatment for spinal cord injuries is neural prosthetics, which replicate patients' lost nerve function. Neural prostheses may be used as artificial body parts or assistive devices that patients may cognitively control. Other assistive devices include wheelchairs and scooters.

Spinal cord injury specialists may collaborate with physical therapists, radiologists, neurologists, urologists, and orthopedists.

Dr Lacey was great in that he was trying to address my 3 problem areas during one appt so he needed to be organized and to the point (which he was). His MA was also great- reassuring me about the health concerns that Dr Lacey could deal with, knowledgeable about setting up the US machine. I felt very reassured during my shoulder injection. We were a bit rushed because of trying to address so many problems and I didn't think to ask questions about exercise modifications after the shoulder injection.The main problem I've had wit PAC Med is being on hold sometimes 45 min trying to schedule an appt.

I've been very lucky to have a doctor who listens to what I have to say. I also appreciate the nurse that comes in before the doctor to talk over what else going on. I like it much better when we're not recording our sessions, I feel that the doctors listen more if you can speak freely to them. I feel it's very important that both sides feel a trust to speak freely and easily. I've been very fortunate to have over 20 years of excellent care with all specific medical center doctors. Dr. Lacey was and has always been ready to take care of my problem and questions that I have had. He does not waste time when he develops a plan for my care be it simple or complex. Not too many doctors approach their care with as much attention as Dr. Lacey. He listens. He reacts. He asks questions. He ensures that I know exactly when his care plan is going to be. If only more doctors approached their craft with the same attitude as Dr. Lacey, we would have more sick person recover faster.

When I came to see Dr. Lacey, I was having troubles breathing. I came to see Dr. Lacey for shots in my knee and shoulder. Dr. Lacey was concerned about my breathing and suggested I go see the primary doctor immediately. Thanks to Dr. Laceys concern. I ended up in the ER where they found blood clots in my lungs, then a hospital stay. Had it not been for Dr. Laceys concern I would have just gone home, and who knows what might've happened. I have enjoyed having an annual checkup with Dr Lacey. I need only to see him if there is a change in my condition. I'm currently at a plateau a stable with regard to my condition. I have been a patient for a couple years now and I have finally had a good and positive relationship with my doctor, he is very attentive and helpful and gives me options but hears me and I like that he doesn't just ignore my needs or my thoughts In my opinion he is a very great doctor.As a matter of fact Pac Med is great in general thanks

Dr. Lacey has always listened to my concerns and taken action to reduce my pain and concerns. This time a slightly different situation was discussed from a previous appointment. But both concerned my buttocks and right leg. And both affect my walking. He explained all reasons and set up a loan for recovery. He is a great MD

At one point I did not think Dr. Lacey cares about what I had to say about a sibling contributing to the stress that is exacerbating the TBI symptoms. Though later I felt he had many knowledgeable things to say and understood my medications might be an issue and the care he prescribed for me and his explanation of things.

Provider was running pretty late to appt. Offered a courteous apology. Felt our time was rushed. Doctor seemed distracted & only partially Interested In what I had to say. His responses to my Inquiries were rote & shallow In nature. This was a very different experience from my Initial visit with him a year ago.

Dr. Lacey was very in tune to my situation and explained things very thoroughly. He initiated a treatment and will follow up on this treatment. I am VERY pleased with his knowledge, care and treatment as a patient. He is very respectful to both me as the patient and to his staff person in the room with me.
