Debridement (Removal of Unhealthy Tissue)

Debridement is a procedure that removes damaged skin to help the body heal a chronic wound, acute injury, or burn. Several layers of skin surrounding a chronic wound can include foreign debris, dead (necrotic) tissue, infected tissue, and calloused (hyperkeratotic) skin. If left untreated, these damaged skin layers can spread infection to other parts of the body and may aggravate the wound and delay its healing.

Debriding (removing) this damaged and diseased skin encourages the growth of healthy new tissue, impeding infection by triggering the body's natural cycle of tissue renewal. For this reason, debridement functions both as a wound treatment and as a preventative measure against more complex consequences of infected skin, such as sepsis and amputation. In some circumstances, debridement provides additional aesthetic benefits such as decreased scarring. Debridement also permits doctors to retrieve tissue samples for testing, such as to determine whether patients need antibiotic treatment.

Wounds that frequently require debridement include:

  • Nonhealing diabetic ulcers
  • Chronic wounds
  • Severe burns
  • Contaminated wounds

Debridement can be performed either surgically or nonsurgically; some methods are less invasive than others. Doctors select the appropriate debridement technique by considering the location, cause, and size of a patient's wound and may also weigh the patient's overall wellbeing, age, and risk of infection.

Surgical debridement is performed by cutting away dead, infected, and hyperkeratotic skin with a surgical scalpel. For deep wounds that tear away the skin and go into the muscle, skin from another region of a patient's body may be grafted onto the wound, which may then be periodically assessed and rebandaged until fully healed.

Non-surgical debridement is achieved with water or a dissolvent gel. Water softens the damaged skin and affected tissue, allowing doctors to remove it with wet-to-dry dressing. When wet tissue dries, it adheres to the dry dressing or bandage and comes off when the dressing is pulled away. In contrast, dissolvent gel deconstructs tissue, which can be cleaned off of the wound. Nonsurgical debridement is less invasive than surgical techniques; however, it can entail repeat treatments, slowing wound recovery.

While the prospect of stripping away skin may seem jarring, debridement can be critical to the treatment and healing of wounds which cannot not heal independently.

Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Shoulder Arthritis

Shoulder arthritis is a very common condition that causes pain, stiffness, or a grinding sensation in the shoulder. Some patients may hear a clicking or popping noise, and all symptoms are more noticeable when moving. Arthritis occurs when the cartilage that cushions joints becomes damaged.

Normally, when two bones come together inside a joint, they are separated by a layer of tough, smooth cartilage. The cartilage allows the bones to glide across each other, which makes movement easier. Cartilage can be damaged by an injury, by an illness such as rheumatoid arthritis, or by simple wear and tear (osteoarthritis). In the shoulder, the two joints most commonly affected are the AC joint (where the shoulder blade and collarbone connect at the outside edge of the shoulder), and the glenohumeral joint (where the arm bone meets the shoulder blade).

Treatment for shoulder arthritis may include:

  • Ice and rest
  • Exercise and physical therapy
  • Over-the-counter pain medications
  • Corticosteroid injections
  • Surgery

Surgical treatments include the removal of inflamed tissue and joint reconstruction. The type of arthritis and severity of the damage will determine which treatment is most appropriate.

I do not feel heard. His focus is on arthritis when I have pain and MRI, indicating other issues. Information regarding the cause to my injury is wrong in the chart. He does not seem to know me or my case from one appointment to the next, And has no urgency in helping improve my condition. I had no pain or Issues with my shoulder prior to my injury, my injury is progressing to the point I can't use my arm for ADLs and is keeping me awake at night with worsening s/s. I feel I have done all the work to get to the option of surgery-and I feel I'm the only one thinking there is something wrong or needing fixed. I am not 100% confident that he is fully trustworthy with my case. And I am looking elsewhere for someone who can know me, care for me and ease my way. Thank you for asking.

In all of my encounters with Dr. Mitchell and the broader team of providers and administrative support within Orthopedics, I have felt respected and heard as a surgical candidate and eventual patient. Examples include demonstrating patience and empathy during in-patient consultations/appointments/follow-up, responding to questions in a thorough, understandable manner, promptly responding to inquiries via MyChart or via phone as appropriate, and following up post-surgery to check-in on recovery. I also appreciate Dr. Mitchell and team's reassurance and genuine investment in my post-op recovery goals.

The health care provider and sports medicine team did an excellent job setting an up appointment for me as soon as possible to care for my injury needs. They were respectful and handled necessary tests (e.g x-ray, MRI, etc.) to help my care right away. One recommendation I hope that can help myself (and others) is having options for diagnostic/diagnosis codes for different procedures to help the clients with their insurance needs. Some codes may not be covered under the clients insurance, so having a substitute code or a similar code for replacement may help the client be covered with their insurance, thus helping them financially. Thank you!

I was not impressed with Dr. Bryan Mitchell. I was there for a follow up visit on my knee. He told me that I obviously was not a candidate for surgery and I was receiving complications from the surgery. He had no valid advice to help me with my knee problems, my back problems and my shoulder problems. He performed a surgery on my shoulder as well as my right knee. He told me I could do a follow up visit with my shoulder surgery, but he did not help me with my knee Surgery postop so I would not recommend this doctor

I have the perception that Dr. Mitchell is satisfied with his career. He listened to my concerns and helped me to understand the steps to treat my injuries. He also appeared to be an expert on surgical procedures and rehabilitation. I am grateful that Dr. Mitchell is treating me, and what is most important to me is that he takes Into consideration my medical condition to ensure the procedures are safe. He was kind, respectful, and professional.

I went to see Dr. Bryan Mitchell for a second opinion on my left shoulder and was impressed with his thoroughness and he listened to my wife and my concerns. He has performed two surgeries on our daughter and I completely trust him, which is why I went to him for a second opinion.He took time to answer all of our questions and go over all potential scenarios and outcomes. I left with a greater sense of peace of mind regarding next steps.

I felt like the recovery process needs more work. The nurse did not make sure my daughter went to the bathroom before we left being we had a long trip home. The process was rushed. Provider did apologize for being behind, but I also felt like they were in hurry to get us out of there in recovery. Also did not explain to me that my daughter could have some shallow breathing after surgery and probably should keep her awake for a while.

I have used providence orthopedics for the last 4 knee surgeries and have had injections on the non surgical knee. I feel like this practice has always taken great care of my medical needs. I love providence orthopedics. My sister will be seeing Dr Mitchell for a surgical consult soon. I feel like their goal is to provide procedures that enhance the quality of a patient's life and I appreciate that.

Dr. Bryan Mitchell was my second opinion. He SAVED me from a false diagnosis. Also, when I first met w him, I had just been diagnosed w breast cancer. He helped me manage my hip issues while I was going through the cancer. He has absolutely risen far & above EVERY expectation. A truly great Doctor. I am so grateful to have him as my Ortho surgeon.

Dr Mitchell is will to listen and take the time to try to understand what is going on. Yes that has a drawl back because they seem to run behind when you have appointment. Even though he was behind he did still take the time to listen to me and make sure everything looked okay. He really care for people and not just the number.
