Blood Transfusion

Blood transfusions typically treat those who experience a substantial loss of blood from an injury or surgical procedure, or those who suffer from conditions that alter the blood supply or blood components. For example, a blood transfusion may contribute additional red blood cells to someone with anemia, a condition where the body does not produce enough red blood cells. Other conditions may include:

Hemophilia (blood clotting disorder)
Leukemia
Liver disease
Sickle cell disease (misshapen red blood cells)
Thrombocytopenia (lack of platelets)

When a patient has sufficient blood volume and lacks one or more components, a complete blood count (CBC) test can help determine if a patient requires a transfusion by measuring four blood components: red blood cells, white blood cells, platelets, and hemoglobin. If any of the patient's component counts is significantly below a standard range, the patient will likely need a blood transfusion. The complete blood count test uses a needle to extract a small amount of blood for testing.

Blood transfusion recipients should be tested for their blood type, which is one of four different types: type A, B, AB, or O. Patients with A, B, and O blood can only receive a blood donation matching their blood type or of O blood, which is known as the universal donor. Those with AB blood are considered "universal recipients"; they may receive A, B, AB, or O blood. Further complicating blood types is the presence or absence of the RhD antigen, adding a positive or negative modifier that should also be considered. Incompatible blood donations may cause transfusion sickness, such as a hemolytic reaction, where the body fights the donated red blood cells.

In certain emergency or trauma situations, medical teams often do not perform a complete blood count (CBC) test or blood type test. In those cases, patients are often given O negative blood, which is universally accepted. Some patients have their blood type on a medical card or bracelet, allowing emergency medical teams to issue compatible blood without testing.

Transfusions are performed by inserting a needle attached to an intravenous line (IV) into a vein in the body. This IV line is attached to a bag of blood that steadily dispenses blood into the patient's bloodstream. The type, volume, and components of donated blood depend on the patient's condition and needs. After the transfusion is complete, patients are generally permitted to resume normal activity, unless prohibited by a condition or injury. Some patients may experience bruising or soreness around the area that the needle was inserted.

At times, a second complete blood count test is performed to determine if the transfusion has brought patients' component counts to normal levels. Some patients may require multiple blood transfusions to reach a healthy blood volume and blood component count.

Once patients' blood volumes and components reach sufficient levels, they will generally begin to feel better. Patients of all ages, from newborn infants to elderly adults, can receive a blood transfusion.

Breast Biopsy

Breast biopsy is a procedure where a small sample of tissue is collected from the breast for testing. A physician may prescribe a breast biopsy for a variety of reasons, but they are most frequently prescribed for a lump found in the breast. Biopsy samples extracted are examined by pathologists in a laboratory setting and are tested to determine if they are cancerous or benign, or indicative of some other condition. Depending on the lab results, physicians may prescribe additional treatment. Other circumstances which may prompt a breast biopsy include:

Mammograms that detect other unusual features in the chest, such as a cyst or calcium deposit
Breast scans or ultrasounds that display abnormalities
Nipple changes, such as scaling, crusting, or bloody discharge

Breast biopsies are collected through several different procedures and the specific type of biopsy that a patient receives depends on the size, location, and features of a breast lump or irregularity. The three main methods of collecting a breast biopsy include:

Fine needle aspiration (FNA biopsy), where a thin needle is directed into a breast lump or abnormality, extracting a small sample of tissue or liquid
Core needle biopsy (CNB biopsy), where a hollow needle is directed into a breast lump or abnormality, extracting cylinders of tissue called "cores"
Open (surgical) biopsy, where a breast lump is removed through an incision in the breast

Fine needle aspiration (FNA) and core needle biopsy (CNB) are sometimes image-guided, where a mammogram, ultrasound, or MRI helps direct the physician performing the needle extraction.

Fine needle aspiration, the most common biopsy method, generally requires few patient preparations. It is recommended patients avoid using lotion, deodorant, and perfume prior to their biopsy procedure. FNA biopsy is performed with local anesthesia (topical numbing cream) and typically has a quick recovery, but patients may experience soreness for a few days. Core needle biopsy extracts slightly more tissue than an FNA biopsy.

Open (surgical) biopsy is more invasive and requires general anesthesia, where patients are put to sleep during the procedure. Patients must fast prior to the surgery. After the biopsy is performed, patients are monitored in a recovery room for a time before being discharged, when they will need someone else to provide transportation.

Although some may consider breast biopsy to be a simple procedure, it can catch critical issues - particularly breast cancer - that impact wellness. If a breast biopsy reveals an underlying condition like life-threatening cancer, patients can receive the diagnosis and medical treatment that is necessary for their long-term health.

Thrombocytosis (High Platelet Count)

Thrombocytosis is a disorder where people have too high of a concentration of platelets. Excess platelets can cause blood to clot in the veins. Blood clots in the veins are particularly dangerous, as they can obstruct blood flow and cause heart attacks and strokes. Thrombocytosis has two forms:

Thrombocythemia, also referred to as primary or essential thrombosis (ET), develops from irregular bone marrow cells that produce too many platelets. The cause of these bone marrow cells is unknown.
Reactive thrombosis (RT), also referred to as secondary thrombosis, arises from a different condition such as cancer, anemia, spleen removal, or infection. It is most common in older adults, but can occur in people of all ages. Symptoms of reactive thrombosis may gradually develop over time and typically include bruising, gum bleeding, and blood clotting.

If a doctor suspects a patient has thrombocytosis, a complete blood count (CBC) or platelet count test helps determine the concentration of platelets in a person's blood at a given time. Thrombocytosis can be diagnosed by hematologists using advanced tests on platelet samples. Flow cytometry tests detect abnormalities by evaluating the characteristics of a platelet sample using targeted lasers and lights.

Patients with high platelet counts are frequently prescribed a daily dose of over-the-counter medication aspirin to thin the blood. Platelet-lowering medications, such as hydroxyurea and Intron A, may reduce the likelihood of blood clot formation. Excess platelets may also be removed from the blood by a procedure called plateletpheresis or platelet reduction apheresis, where blood is drawn, circulated into a centrifuge that separates the blood components, and returned to the body with some platelets extracted. Plateletpheresis is a rare procedure reserved for patients who have severe clotting.

Patients can also modify their lifestyle to better manage their disorder and minimize complications. Patients with thrombocytosis are advised to drink plenty of water, exercise regularly, and to avoid smoking. Many people with thrombocytosis live healthy and fulfilling lives with proper treatment and precautions.
