Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

  • A type 1 diabetic whose pancreas has been damaged will need insulin.
  • A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
  • Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Dr. Schreck is a cardiologist so he is a specialist to me, not my primary. But I feel lucky and blessed that he was the cardiologist I got to see when heart rate went to 178 and I came into St Joseph's emergency room. I will stay with him forever. He has been patient and explained everything, but since AFIB is new to me and I've never experienced before what I am experiencing now, I do have a lot of questions and while he is patient and explains, I also feel hurried out before the end of our visit. I realize that he has other patients to see and that as this condition becomes routine to me, I won't have a lot of questions, but this is the only thing that is less than perfect for me. On the other hand, the one time I asked a question online during the waiting period to next appointment, he answered it online, so I was impressed and I'm grateful and happy to have him taking care of me. I don't think I could have anyone befter.

From start to finish it's always a good visit with Dr. Schreck. The receptionist was very friendly. I love Dr. Schreck and even his assistant is very good, last but not least scheduling an appointment for six months is easy and the woman at that desk is always very friendly as well. They never keep me waiting. I had an appointment for a bone density test prior to going to see Dr. Schreck and I arrived a half hour early. They didn't keep me waiting even then when I was early. I would highly recommend Dr. Schreck and all that work for him.

Everything about Dr. Schrek and his team were perfect. Taking this survey which has too many unnecessary questions, should have all the questions on the same page, and I probably won't take this survey again, if I didn't like the care I received with Dr. Shrek and his wonderful team, I wouldn't be making an appointment to see himRespectively, not sure I think surveys are very helpful to the hospital, and respectively that's it for now, was 5-7 minutes, was that worth it to you??

Dr. Shrek was incredibly interested in my current health concerns. He understood this was a second opinion, and being so, tried to explain in great detail why his opinion different from my original doctor. In the future, if I have a choice and this current matter has been put to rest. I would ask that this doctor be my cardio guy. He was truly understanding, sympathetic and devoted to spending enough time so I understood what was going on.

My visit was for re-enforcement for changing a prescription dosage as suggested by my primary care physician. This is only the 2nd time I have seen Dr. Schreck. I provided him with test results secured by the cardiologist I have seen since the late 80's. He is a very good listener. I am considering having him as my cardiologist instead of my other doctor - because he is highly recommended and is closer in proximity.

It was very refreshing to find a a doctor that genuinely cared. He took the time to answer my daughter's concerns and I really appreciated it. She was asked if she wanted to come in with me for the visit. That in itself was a pleasant surprise. I am so glad to have doctor Shrek as my cardiologist, he gave us plenty of good advice and recommendations for moving forward with my conditions.

I like Dr. Schreck! Saw him as a referral through the Providence network. He was attentive, respectful, informed, and he communicated in a relaxed and clear manner. Very easy to listen and relate to. All other staff were excellent as well. From reception to nurse to appointment staff, each of them were helpful and pleasant. Tremendous customer service!
