Addictions

Addictions are chronic diseases of compulsive or uncontrollable behavior which may have adverse health effects. Addictions can include substance abuse (such as the abuse of opioids, alcohol, or prescription drugs) and certain non-drug related behaviors, such as gambling or video games. People with addictions may experience an uncontrollable urge to use substances or perform certain activities, although these behaviors may be harmful to themselves and others.

Those suffering with a substance abuse addiction are often unable to cease their behavior without some form of medical intervention. Studies have found that repeat use of certain drugs can figuratively rewire the brain, resulting in alterations to neural pathways associated with dopamine. This means that repeat drug users may become reliant on consuming the substance in order to function. For example, those with alcohol addiction ("alcoholism") become physically dependent on alcohol and may experience extreme withdrawal symptoms when avoiding alcohol entirely; in rare cases, medically unsupervised withdrawal can lead to death.

Behavioral addictions are similar to substance abuse addictions; however, people with behavioral addictions are addicted to a behavior or emotion associated with certain behaviors, as opposed to drugs. In addition to gambling, common behavioral addictions include food, video games, social media, shopping, and pornography addictions. When performing the activity, people experience euphoria (a rush of dopamine) that slowly lessens over time, meaning they build a tolerance and dependency on a behavior. For example, people who are addicted to shopping may need to purchase more items or spend more money to feel the same rush that once fueled their addiction.

Addictions can be assessed through a variety of means such as diagnostic screening, abuse assessment, or psychiatric evaluation. Treatment can help patients recover from their addictions and manage mental and physical health conditions related to their addiction. People with addictions may be treated in rehabilitative treatment facilities, hospitals, and other clinical settings.

Addiction treatment for substance abuse may include:

Detoxification (withdrawal from an addictive substance)
Pharmacotherapy (prescription medications), such as buprenorphine-naloxone (Suboxone), disulfiram, and naltrexone to ease withdrawal symptoms
Overdose treatment, such as the administration of naloxone (also known as Narcan)

Addiction treatments for both substance abuse and behavioral addictions may include:

Addiction therapy and psychiatric care, such as cognitive behavioral therapy (CBT)
Prescription medicine for co-occurring psychiatric disorders
Twelve-step programs and support groups, such as Alcoholics Anonymous (AA) or Food Addicts Anonymous (FA)

Addiction treatment has expanded to include care for patient substance abuse in non-clinical settings. A number of community programs now provide at-risk substance users a safe place or means to use drugs and mobile doctor's services for drug-associated illnesses and injuries. Behavioral addictions are also gaining increased attention as researchers and mental health professionals seek to better understand and treat them. For example, some researchers are assessing the connections between video game addiction and childhood risk factors. More and more, mental health experts are recognizing the importance of meeting people with addictions where they are at, using more advanced treatments. Addiction treatment is intended to care for people with addictions with compassion and respect.

Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

He seems to get conversations mixed up along with what he's saying about prescription then when questioned, makes you feel like you're the one who's not clear on what you heard. As a patient who likes to think she's pro-active with her care suggestions and seems to know what does and doesn't work for her, being taken more seriously and listening (in my case reading) what I have to say is very important in gaining my confidence and trust. Showing signs of being irritated for asking clarification doesn't gain my trust. Having me go through procedures I have tried and failed despite vocalizing this also doesn't gain my confidence. I would prefer to seek another clinic where patients are listened to and encouraged to take an active role in decision making. All patients should be treated fairly and equal.

As I have continually said on all the surveys from you for Doctor Rudolf, he has given me my life back. I He was highly recommended to me by a friend and her daughter. I hesitate because it is in Seattle Swedish Pain Center. I finally went to see him in 2012. He immediately put me in treatment for the many medicines. One was fentanyl; I was getting severely ill from the prescription so he put me in-patient immediately, I was detoxed and given better medications and started acupuncture. It took some time but it was and still is the best treatment I have ever had. He is the best doctor who took me from a 10+ pain 7x24 down to maybe a 3 during the day and a little higher in the evening, but the medication therapy he has me on kicks that pain down to a one. Doctor Rudolf has saved my life, he gave me life back, and I will be forever grateful.

Dr. Rudolf is an excellent physician! He's kind, knowledgeable, meticulous and respectful! He has a broad understanding of how to treat chronic pain, and uses many different methods and approaches to tackling this condition. One of my favorite things about Dr. Rudolf is his use of acupuncture to treat pain. He's very skilled as an acupuncture practitioner. It's unusual for a practitioner of contemporary Western medicine to incorporate such techniques in their repertoire, and it really highlights his openness to all aspects of healing as well as his commitment to helping his patients. I feel extremely well cared for by Dr. Rudolf, his nurses and his entire team. I highly recommend him as a physician.

I appreciate the way I have been cared for as my prescription meds changed due to manufacturing and supply issues. Just when you think it's under control for another month something comes out of the blue, substitution from the pharmacy that caused reactions and need for more prescriptions. I can't say enough about Dr. Rudolf keeping up with the twists and turns and providing help where he can. Pain control is a major complication in the system from doctors to pharmacies with some showing actual concern and others seemingly not caring. Dr. Rudolf has proven to care for me and provide what I needed . Much appreciated!

Dr. Rudolf is the best pain specialist I've ever worked with, and I'm grateful to be his patient. He overhauled my fibromyalgia medication and helped me find a combination of medicines that lets me function almost as if I weren't even sick. The only thing consistently wrong with this practice is that they are constantly running late on appointment days and it's hard to get an appointment beforehand, because Dr. Rudolf tries to fit in so many patients. Knowing how many people need a good pain specialist and how rare a doctor like him is, I can't really fault him for that.

Dr Rudolf isnt always 100% on time, but he is worth the wait as he provides high quality appts to myself and I'm sure others. He always asks questions about health and background questions to understand the the full picture of my pain and health situation and treats me as a whole person. He knows my family background and asks how they're doing, and encourages me in my general wellness journey where he can, especially where it can help my pain. We arent always simple patients and he takes the time to get to know us and pur whole journey which takes time

Judging, esp., is very friendly and a pleasure to see & deal with @ the front desk. The MA who brought me in was very nice & also efficient getting details of visit & updated history. I think Dr. Rudolph absolutely rocks. I have an incredible amount of respect for him. The office staff Is very good. They are always attentive, kind, helpful and extremely professional. I don't know what today might look like if, many years ago, fate had not landed me in the care of Dr. Rudolph during an ugly, dark time. He actually might have saved my life or at least kept me from falling Into an Inescapable catastrophe. So yes, my hat is always off to Dr. Rudolph and gang.

Any and All experiences with Dr. Rudolph have been with the utmost of professionalism !!! He is and has been the best of ANY care provider that I've seen at Swedish, or ANY other healthcare provider ! Which I admit, are quite numerous even in Seattle and King county. Dr. Rudolf was inviting, calm, and informed. He asked appropriate questions to get to the root of my concern, and offered a variety of treatment possibilities, always letting me know the decision was mine. He clearly is an expert in his field, and I was treated on site, with great care and left feeling hope for the first time in years. His entire staff was respectful, and even humorous at times which made me feel so welcome. Definitely returning for this next level care.
