Adrenalectomy (Adrenal Surgery)

Adrenalectomy is a surgical procedure to remove the adrenal glands, which lie above each kidney. The adrenal glands are responsible for producing hormones that are necessary for bodily functions, such as the regulation of blood pressure and metabolism. If the adrenal glands overproduce certain hormones, or if a tumor grows on them, removal of the glands is a treatment option.

Adrenal surgery can be open or laparoscopic. Laparoscopic adrenalectomy is a minimally invasive procedure that inserts a thin tube with a camera (laparoscope) through several small incisions on the abdomen. The surgeon uses the laparoscope to guide the procedure and is able to access and remove adrenal glands through the small incisions. Laparoscopic adrenal surgery lasts approximately 1-2 hours.

Laparoscopic adrenalectomy is associated with shorter recovery periods, reduced risk of infection, and smaller scars. Although these factors may be desirable, the location of the tumor, its size, and the severity of the condition may require that an individual instead undergo open surgery. An open adrenalectomy is typically done for larger adrenal tumors and requires a single, large incision on the abdomen. Open adrenal surgery takes around 2-4 hours.

Following the operation, patients will remain in the hospital for 1-2 days for a laparoscopic adrenalectomy or 3-5 days for an open adrenalectomy. Patients will likely have a liquid diet immediately after their surgery, but should be able to return to solid foods after a day. If both of the glands are removed, patients will be given drug therapy to replace the hormones they produced.

Radical Prostatectomy

Prostate cancer is uncommon in men under 40, but the risk of getting the disease significantly increases after the age of 50. If doctors suspect their patients have prostate cancer based on a physical exam or blood test results, he or she may recommend a biopsy. This diagnostic procedure collects tiny samples of tissue from the prostate, typically with the use of a special needle. The samples are then examined in a laboratory to determine whether any abnormal cells are present. Biopsies of the prostate are most often done transrectally, or through the rectum.

If a biopsy shows the presence of prostate cancer, doctors will discuss available options, such as watchful waiting, radiation therapy, hormone therapy, chemotherapy, and surgery. Together, the doctor and patient will develop a treatment plan that best fits the patients' condition. If a doctor recommends surgery, the patient will likely have a radical prostatectomy. There are three ways this prostate removal procedure can be performed:

  • Open radical prostatectomy, which uses a large incision, usually in the abdomen.
  • Laparoscopic radical prostatectomy, which requires several small incisions in the stomach. A long, thin tube with a camera is inserted through one of the cuts. The surgeon uses this instrument to view the inside of the belly during the operation.
  • Robotic radical prostatectomy, which uses the above laparoscopic techniques but with the help of a robot.

While incisions for the laparoscopic and robotic approaches will always be on the lower abdomen, there are two possible incision locations for the open procedure. More commonly, the cut will start just below the belly button and end at the pubic bone. The other possible but less frequently used incision location is the area between the base of the scrotum and anus. After the cut is made, the surgeon will separate the prostate from the tissues surrounding it. Extra care will be taken to cause as little injury as possible to the blood vessels and nerves. Patients will wake up from surgery with a small pouch that drains fluids from the belly and a long tube that drains urine from the bladder.

Radical prostatectomy is recommended for cancer that is contained within the prostate. If cancer has spread to other parts of the body, non-surgical treatments like chemotherapy, radiation therapy, or hormone therapy may be more appropriate.

The average hospital stay is one day for a laparoscopic or robotic radical prostatectomy, and one to three days for an open radical prostatectomy. Patients are advised to stay in bed until the morning following the operation. Strenuous activities like running and heavy lifting must be avoided for four to six weeks. Although radical prostatectomy aims to remove all of the cancer cells, patients will still need to see their doctor regularly after surgery to make sure that the cancer does not return.

Shock Wave Lithotripsy

Stones are formed when mineral deposits build up and harden in an organ or gland. For example, urine contains minerals that can form crystals and result in kidney or bladder stones. Stones can also occur in other parts of the body, like the mouth. Salivary gland stones form when the chemicals in the saliva harden and get stuck in the tiny ducts of the salivary glands. Pain and swelling are the most common symptoms associated with stones.

Lithotripsy is a procedure used to break up small stones. Shock wave lithotripsy, the most common type of lithotripsy and least invasive way of treating stones, does this using high-powered sound waves. This procedure can treat many types of stones in the body, but it is most frequently used for urinary stones, such as kidney and bladder stones.

Shock wave lithotripsy can be done in one of two ways. Conventionally, this procedure is performed when someone is sitting in a tub of water, and the sound waves travel through the water to their body. A more recent alternative is to send the shock waves through cushions while a patient is lying on a table. Although shock wave lithotripsy is not surgery and requires no cutting, the patient will be given anesthesia before the procedure. Their body should be able to pass the broken-up urinary stones naturally in their urine after treatment. For other types of stones, another minimally invasive procedure may be required to remove stone fragments left by shock wave lithotripsy.
