Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

Age
Gender (most heart attack sufferers are male)
A family history of heart attacks
Smoking
Obesity
High cholesterol
High blood pressure
Diabetes
Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Dr Ehrich is kind, patient & thorough, and has been my cardiologist for many years. During my exam, he found a serious problem which stunned both of us. He took immediate action and walked me down the hall to another doctor to schedule a procedure. We were told this doctor was booked solid for the next month but the seriousness of Dr Ehrich changed all that and I had an appointment within a few days. This new Dr was great, explained everything and answered my questions. However, afterwards I had a panic attack and anxieties of the unexpected came rolling in. I relayed my fears to Dr Ehrich on his portal and he personally called me within an hour and calmed me down. I was fine after our conversation and I'm ok to proceed with our plans.

Dr. Erich and his assistant are the best, The Echo Cardio guy is excellent as well. There have been times in the past when it was urgent to get an appointment with Dr. Ehrich and the office team was not that helpful. This was over 1 year ago. After that Dr. Ehrich advised me should that happen again to tell the person I'm speaking with to come to him directly so that he can fit me in his schedule. To be clear things have improved since that issue over 1 year ago. Thanks for the survey.

Dr. Erich is amazing and, hands down, my favorite medical provider. He listens, he talks things through, and he is very thorough both in his evaluation and in answering all questions I bring to him every visit. I can't say enough good things about him. And to make things even better-his staff is top drawer as well. Every visit to Dr. Erich is one I look forward to, as the care I receive is second to none. I'm very grateful to have him as my cardiologist.

I am extremely lucky that the office was able to get me in. Dr. Mark Erlich is exemplary. I saw him for a second opinion and care. The first cardiologist I saw ordered tons of tests and did not explain them. She made me feel like I was going to die tomorrow. Dr. Erlich gave me clear instructions and explained everything. I sent a message through the Portal and he answered immediately. I left the office feeling safe and care for!

Dr. Erich has been my cardiologist since I arrived at my new home about 12 years ago. During that time I have felt my care has been excellent. He has explained issues with my atrial fibrillation and been very attentive to any tests I might require. He listens to me when I have questions and concerns. Also, he's very down-to-earth and affable. I'm very happy with him and the care he offers.

I brought up information about how a different doctor (from a different hospital dealing with a different health issue) told me to cut back on the amount of a pill to take, and Dr Ehrich (Providence provider) understood and told me that the issue can also be addressed with a lower amount to take of a pill he prescribes. I was very happy to be able to communicate and have this addressed.

His knowledge and experience is truly amazing, and delivered in a easy to understand manner, in good humor, smiling, focused, and willing to provide follow-up as needed after regular office hours.
