Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

A type 1 diabetic whose pancreas has been damaged will need insulin.
A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Lung Issues

Every cell in the body requires oxygen to function and produces carbon dioxide as a waste product. In order to keep the body's tissues constantly supplied with fresh oxygen and to keep waste from building up, the lungs pump air in and out, even while sleeping or unaware. Unfortunately, diseases and disorders affecting the lungs are some of the most common medical problems afflicting people around the world. Some of the most common lung problems include asthma, COPD, pneumonia, and lung cancer.

Asthma is a chronic disease where swollen, inflamed airways make it hard to breathe. It is often brought on by specific triggers. While asthma is a common condition, affecting millions of people, it can also be serious and even fatal in severe cases. Treatment involves avoiding triggers and using inhaled corticosteroid medications.

COPD, or Chronic Obstructive Pulmonary Disease, is the name given to two illnesses that both cause difficulty breathing: emphysema and chronic bronchitis. In emphysema, the air sacs of the lungs become damaged, while with bronchitis they become clogged with mucus. COPD is the fourth leading cause of death in the United States. It is almost always caused by breathing irritants into the lungs, such as smoking or heavy pollution.

Pneumonia is a lung infection that can be caused by bacteria, a virus, or fungi. Most healthy people recover from pneumonia in a week or two, but for some people pneumonia can be fatal. There are treatments available for pneumonia, but the best treatment is to prevent getting sick in the first place. People who are at risk should make sure they get a flu shot every year. There is also a vaccine available for the type of bacteria that causes pneumonia.

Lung cancer occurs when abnormal cells inside the lung grow into a tumor, destroying healthy tissue. Because it is so difficult to detect lung cancer in early stages when it has few symptoms, it is the deadliest form of cancer, responsible for the most cancer deaths both in the U.S. and worldwide. Most lung cancer is caused by exposure to tobacco smoke, but between 10-15% of cases are not linked to smoking.

Some problems with the lungs are minor and are not cause for concern. However if symptoms have lasted for a month or more, even if they are minor, it is worth checking with a healthcare professional. A cough, shortness of breath, or excess mucus production that lingers for weeks are all worth getting looked at.

Lyme Disease

Lyme disease is a condition caused by bacteria (borrelia burgdorferi) that are transmitted via infected ticks. Most cases of Lyme disease are found in the United States East Coast and parts of the Midwest. The three main vectors of transmission for Lyme disease are:

Blacklegged (deer) ticks, which spread Lyme disease in the Northeastern, North -Central, and mid-Atlantic United States.
Western blacklegged ticks, which spread Lyme disease in the Pacific and Coastal regions of the United States.
A mother infected with Lyme disease may pass it onto a fetus.

As background, ticks lay on the tops of grass and brush, clutching onto leaves with their legs and with their arms outstretched. This formation is called "questing" and allows ticks to quickly latch onto passers-by. Within two hours of biting a person's skin, the tick slowly begins to feed from the blood of its host, which can transmit the bacteria that causes Lyme disease. This tick activity generally occurs seasonally and tick bites mostly occur during the springtime.

After being infected, people with Lyme disease may develop symptoms like fever, exhaustion, headache, joint aches, and skin rash, usually three to thirty days after the tick bite. Lyme disease is characterized by a particular skin rash called erythema migrans, which is an enlarged red rash that starts at the site of the tick bite and expands outwards. The rash is a strong indicator that someone has developed or is at risk of developing Lyme disease.

In the following few months, symptoms may worsen and progress to include:

Facial palsy (facial droop)
Headache and neck pain
Arthritis
Nerve pain and numbness
Spinal and brain inflammation
Abnormal heart rhythm (Lyme carditis)

Most cases of Lyme disease dissipate over a few months. Lyme disease may be cured by a course of oral antibiotics, such as amoxicillin or doxycycline. Other treatments may include:

Nerve blocking drugs for pain, such as gabapentin
Topical ointments for rashes

People can take action to prevent Lyme disease by exercising caution when in environments with ticks. Using insect repellant and wearing long socks and protecting clothing can help prevent tick bites. Outdoor enthusiasts should regularly perform "tick checks," evaluating their skin for small ticks, especially in areas where ticks may easily burrow. Showering within two hours of returning indoors also can reduce the likelihood of developing Lyme disease.

I had called and left a couple of messages to set up a follow up appointment with Dr. Troum. No one called me back. I had to make a third call before someone finally got back to me. This spanned over 2 month period. They wanted to set an appointment for Jan. 2023 and by this time I mentioned that my goal for insurance purposes was to have the appointment before year end. So on a good note, they did accommodate this request. This is my second time seeing Dr. Truom. The first time was when the pandemic first started so we weren't really able to get to know each other because the follow up was done virtually. This time however, 2 years later, I really got a better experience with him and it's very obvious that he's super knowledgable one of the best in his fields...which might explains why it was tough to get an appointment. Nevertheless, I think someone dropped the ball if it took almost 2 months to get appointment. Since my life is very busy at work. The last thing I want is rework. I'd rather if things just get done without me persisting for 2 months. I love doctor Troum and I respect how he treats his staff. He provides his office with fresh flowers weekly. The assistant that was in the room with him was also very on her game. It's just the people who deal with the scheduling that has opportunity for improvement.

Dr. Troum is one of my favorite doctors and trust me when I say I see at least 15 different practitioners yearly. He's extremely knowledgable but most importantly, he knows how to explain things in a clear and easy to understand manner. He's incredibly proactive (ex: I asked him whether or not you can overdose on B12, the answer was yes and he proactively ordered some additional tests to see where my B12 level was and sure enough, it's too high; as such, I have since stopped taking my B12 supplement). He is also very personable and has incredibly great people skill, you don't feel like you're just one of many patients, you feel like he's your buddy. Even though I probably see him at most twice a year for my routine check-up, I feel like my relationship with him is as good as my relationship with my primary care doctor who I see probably every other month. I love Dr. Troum and wish every doctor I see is as good as he is. I cannot give enough praise and will be very sad when he retires. He truly is the bomb when it comes to quality doctors and I feel very lucky to be one of his patients.

Dr. Orrin Troum is the highest quality provider of superb medical services. His knowledge is wide-ranging and promotes absolute confidence. This motivates the patient to follow medical advice and continue to seek on-going assessment and care. Dr. Troum is not only completely efficient in conducting interviews and advising based on that information, he possesses a rare capacity to interact on a human level with his patients. This is no small matter for those in distress and in need of support. Dr. Troum really knows his patients. This was the best doctor appointment I have been to in my life! Dr. Truom made sure I was actively involved in my own care. How novel, right? He sat with me and reviewed previous labs, educated me on things outside of his discipline even, and genuinely seemed to have a vested interest in knowing everything about my current condition. I don't know what my labs will reveal, but I feel confident I will be in the best hands if I require more treatment in the realm of Rheumatology. Thank you for your compassion, Dr. Truom! I am a Healthcare Advocate, and my experience at your office was the way medicine really should be!

Best doctor/patient care visit I have ever had!!Dr. Troum took a tremendous amount of time & care in my visit (treated me like I was his only patient). He is perceptive, excellent at detecting things on examining joints, he listens carefully, he explains things clearly, he reported important results from previous tests that I was not aware of, he is compassionate, funny, interactive & caring. All the people in this office, from his in-take man, assistant in the room with him, to the blood-draw technician were across the board terrific!I have already recommended him to a number of people.

Dr. Troum is an excellent clinician and has a wonderful bedside manner . My medical situation has grown increasingly complicated over the last month, due to my being diagnosed with a fib. I was hospitalized for two days at the beginning of the month as a result of a traumatic fall, resulting in a split skull, staples in the wound and two days of intensive testing. Plus, I'm now taking medication's for the heart condition. In addition to the RA. So it's important that both teams of doctors are aware of all the medication's. My experience with all of the physicians has been excellent.

As always, Dr. Orrin Troum continues to demonstrate the finest qualities professionally and personally. His respect for patients is obvious and he is a superb listener, thereby making medical decisions specific to patient needs. I arrived hours early to my regular 3 month appt to see Dr Troum due to my mistake . His receptionists scrambled to get me in early and fortunately some of his patients were running late so Dr Troum accommodated me 3 hrs early!!! And I have never felt rushed in his office. After seeing three other Rheumatologists, I was blessed with being referred to Dr Troum by a friend. I always look forward to my appointments! He is so knowledgeable, thorough and incredibly caring and kind.

Everyone searches for a doctor like Dr. Troum, and we are lucky to have found him. There may be many 5-star recommendations on your website but they don't do him justice. He Is the most knowledgeable rheumatologist I have met as well as being positive and uplifting. We really like how he put things in layman's terms so that we could understand what's going on with my condition. He didn't have the best news, yet he was still able to come up with a good plan and give us hope!
